I have reviewed the following disqualifying conditions: Coronary heart disease with symptoms; heart attack within the last six months; those with an implantable pacemaker or cardio-defibrillator; history of stroke within the last 12 months; poorly controlled type-2 diabetes; insulin-dependent/type-1 diabetes; epilepsy; active tuberculosis; hepatitis, chicken pox or other communicable diseases; individuals with any viral respiratory infection; severe arthritis; severe asthma or restrictive airway disease; chronic lung disease requiring supplemental oxygen or requiring hospitalization/emergency room visit within the last six months; movement requiring a walker or wheel chair; casts (leg, foot, or arm cast); blindness; deafness; poorly controlled high blood pressure; use of blood thinners; any known pregnancy regardless of weeks gestation; body weight of more than 300 pounds; any ambulatory condition or limitation that presents an inability to walk up/down a stairwell without difficulty; unstable gait; younger than age eight; older than age 75.

I understand that Ship’s Medical is not authorized to issue, disburse, or manage the medications of non-crew members without prior type commander credentialing. Visitors and sponsors should ensure all required medication is brought onboard for each individual.

I understand that no bags, open toe shoes, shorts, tops which expose the shoulder or rib cage, and hats or other accessories which might pose a risk to aircraft will be allowed on board. Clothing should be in good repair and have no markings or content which depict drugs or other prohibited paraphernalia.

I will provide my ID type and Number (Drivers license, SSN, State ID, or Proof of Naturalization) to my sponsoring Sailor. I understand I am NOT able to attend Friends and Family Cruise if I fail to provide this in a timely manner.

During initial registration, every Sailor is limited to two (2) guests. We will approve additional guests on a case-by-case basis until the ship is at capacity.

The underway will last approximately TEN hours, depending on your assigned boarding time. Expect traffic upon departure and plan accordingly.

Continental breakfast items will be provided, as will a catered lunch and some snacks. Other food items and drinks are available for purchase from the ship’s store.

Fun Fact: All profits from the ship store are used to fund the crew’s morale events!

Aircraft Carriers experience much less movement than smaller ships in the open ocean but if you are sensitive to sea sickness, you may still feel unwell. Please bring your own seasickness medication, as ship’s medical is unable to issue it to our guests.

Aircraft Carriers are an industrial environment and there is always a risk of injury. Help make this a fun and safe adventure with a few easy steps:

-ALWAYS use handrails when moving up and down ladderwells.

-Do not touch the doorframe or edge of any hatch.

-If a hatch is slamming open or closed, do not try to stop it, just move out of the way and wait until it stops moving.

-Do not use your phone or camera while walking.
